Histopathology report

• indication for investigation - primary diagnosis, staging, relapse/progression, re-staging.

• type of biopsy - excision, needle biopsy, endoscopic biopsy, bone marrow biopsy, extra-nodal resection, splenectomy or other biopsy.

• site and size of - lymph node, skin, bone marrow trephine and other extra-nodal biopsies.

• size and weight of spleen.

• tumour type - lymphoma or others. If lymphoma specify type using a combination of clinical features, morphology, immunohistochemistry and cytogenetics as necessary to characterise entities included in the WHO classification. In general, Hodgkin's or non-Hodgkin's lymphoma, and characterise the latter as nodular or diffuse, small cell or large cell, low/intermediate or high-grade, and B, T, NK or ALCL.

• bone marrow - involved or not involved.
