When the Pneumonia Doesnt Get Better

In patients who are poorly responsive to adequate antibiotic treatment, alternative diagnoses must be explored. Unusual pathogens and mycobacterial infection must be rapidly ruled out, whenever possible by fiberoptic bronchoscopy. Noninfectious disorders—which may mimic pneumonia—must be considered, such as pulmonary infarction, cryptogenic organizing pneumonia (previously referred to as idiopathic bronchiolitis obliterans organizing pneumonia, or BOOP), vasculitis (Wegener's granulomatosis, or Churg-Strauss Syndrome), bronchocentric granuloma, mucous plugging with infiltrates in allergic broncho-pulmonary aspergillosis, idio-pathic acute eosinophilic pneumonia, chronic eosinophilic pneumonia, and carcinoma (mainly bronchoalveolar carcinoma). Cavitary lesions suggestive of pulmonary abscess in fact may be excavated primary pulmonary tumors, or vasculitis (Wegener's granulomatosis).
