This chapter seeks to provide you with an overview of the potential effects of testosterone (T) deficiency in aging men. Although we acknowledge the age-related reduction in serum levels of the adrenal androgens, dehydroepiandrosterone (DHEA) and dehydroepi-androsterone sulfate (DHEA-S), the chapter focuses on the much more potent testicular androgen, T and its metabolites, dihydrotestosterone (DHT) and estradiol (E2). Many of the symptoms and signs of aging and testosterone deficiency are shared. Although investigators frequently administer T to aging men in an effort to differentiate symptoms and signs related to aging from those caused by T deficiency, a large definitive study assessing efficacy and risk is lacking. Thus, we seek to provide you with our current understanding of the effects of T deficiency in aging men, using insights from small, nondefinitive clinical trials.
