Over the past decade, significant progress has been made toward refining definitions of frailty, and toward the understanding of the biology that may underlie frailty. Continued refinement of clinical phenotypes and the development of a standardized definition of frailty through contributions of individual domains and through cluster analyses will be critical to compare individuals in large population databases and to better understand the biology of frailty (Walston et al., 2005). This may be best addressed through studying clusters of signs and symptoms that may be supported by common biology, and utilizing systems biologic methodologies to better incorporate important between-systems interactions (Fried et al., 2005). In addition, development of animal and cellular models, along with the development of large population databases, and the infrastructure to support large-scale analyses, will be helpful in developing the next generation of frailty research and ultimately inform the development of interventions to improve the quality of life in this subset of most vulnerable older adults.
