Cholinomimetics are the senior citizens of glaucoma therapy, and the mechanism underlying their efficacy has been considered to be the most straightforward of all the drug classes used. Although they have been supplanted by newer medications over the past three decades, they are still useful in carefully selected patients, such as open-angle glaucoma patients who are presbyopes with clear lenses or pseudophakes, and angle-closure glaucoma patients.
