For prophylaxis or treatment of thrombosis in HIT, the recommended initial dose of argatroban is 2 mg/kg/min (Table 4). Because hepatic impairment decreases argatroban clearance, a reduced initial dose, i.e., 0.5 mg/kg/min, is recommended for patients with at least moderate hepatic impairment, defined as a Child-Pugh score >6 (Swan and Hursting, 2000).
