Many of the same parameters that predict outcome for primary chemotherapy predict outcome for salvage therapy. In addition to primary site and serum marker elevation, various investigators have suggested prior best response, degree of cisplatin resistance, and the presence of brain metastases as important prognostic factors for salvage chemotherapy. Currently, many investigators divide patients into poor-risk patients with recurrent disease and good-risk patients with recurrent disease, the former group consisting of those patients with prior incomplete response to primary therapy, mediastinal primary nonseminoma, and high hCG levels. Investigators at Memorial Sloan-Kettering Cancer Center divide patients by these parameters to assign either treatment with experimental high-dose therapy or more conventional salvage treatment with cisplatin, ifosfamide, and paclitaxel and standard high-dose therapy.
