Low incidence of clinical side effects such as edema or carpal tunnel syndrome. The number of subjects and the duration of treatment are still small, however, and it is also still not clear whether this generally favorable experience reflects the qualitative differences between GHRH and GH, or simply the differences in the effective potency of the doses used. As in the pediatric setting, there has been no direct comparison between GH and GHRH treatment in doses that produce similar increments in circulating GH.
In carpal tunnel syndrome, repeated hand movements, such as typing, inflame the tendons that pass through the carpal tunnel, which is a space between bones in the wrist. The swollen tendons compress the median nerve in the wrist, causing pain to shoot up the upper limb. Surgery or avoiding repetitive hand movements can relieve symptoms.
Approximately 70 of all tendon injuries are sports related. If the elongation of tendon is less than 4 in an activity, the tendon will return to its original length when unloaded. Above this strain level, however, crosslinks between the fibers of a tendon may begin to fail and some fibrils may rupture. Higher loads such as occur in an accidental fall result in the macroscopic rupture of the tendon. Examples of tendon disorders include achilles tendinitis, caused by running, patellar tendinitis, the result of running and jumping, and carpal tunnel syndrome of the tendons of the wrist and the fingers and tennis elbow, from the overuse of wrist extensor muscles. In most cases, resting the tendon may alleviate the problem. The treatment of tendon injuries may prove difficult because of the need to preserve the balance between resting the injured tendon and preventing atrophy of the surrounding muscles and joints.
Studies of GH administration in patients with adult-onset GHD have shown results consistent with those in childhood-onset patients. Initial short-term studies have shown GH-induced increases in bone turnover, while subsequent longer-term, often noncontrolled studies have demonstrated improvements in bone density. Bengtsson and coworkers have published three studies of GH administration in patients with adult-onset GHD that have included evaluations of bone turnover markers and bone density. The first of these involved 10 patients and demonstrated GH-induced increases in osteocalcin and the aminoterminal propeptide of type 3 procollagen within 6 wk with further increases noted at 6 mo (42). A subsequent study by this group involved 25 patients who participated in a 6 mo, placebo-controlled trial of GH administration, followed by an open-label trial for a total of 12 mo of GH treatment in 12 of the patients (43). At 6 mo, significant increases in bone formation markers were seen,...

CMT disease is a common inherited neuropathy with two major forms distinguished by whether the myelin (type 1 CMT1 ) or the axon (type 2 CMT2 ) is primarily affected. Both CMT1 and CMT2 are genetically heterogeneous with some 35 linked loci, and 25 of the corresponding genes having been identified CMT is characterized by symmetric slowly progressive length-dependent neuropathy, which manifests as a distal weakness of the legs progressing proximally. CMT1 can be distinguished from CMT2 by electrophysiological studies that reveal slowed nerve conduction velocities (NCVs) and neuropathology showing onion bulbs. The most common form of CMT1 is that associated with the CMT1A locus mapping to 17p12. HNPP is an episodic, asymmetric neuropathy, which may be preceded by an antecedent event, such as motor nerve compression or trauma. Presentations may include pressure palsies or focal neuropathy and sometimes carpal tunnel syndrome. The electrophysiology may show conduction blocks, whereas the...
