Phytoestrogens

Dietary phytoestrogens, plant substances that are structurally or functionally similar to estrogen, have been proposed to act as estrogen antagonists in breast, prostate, and endometrial cells. However, epidemiological studies of dietary phytoestrogens and breast cancer have been inconclusive. Some studies indicate that soy intake may reduce a woman's risk of premenopausal breast cancer 77-79 however, at least one study found no association between dietary soy and breast cancer.80 One study has also reported a decrease in risk for postmenopausal breast cancer.79 Ingram et al.81 re-examined the association between phytoestrogens and breast cancer by measuring urinary excretion rates of two classes of phytochemicals and found that an inverse relationship exists with the risk of both pre and postmenopausal breast cancer and urinary excretion. It remains untested whether the measured phytochemicals actually serve as markers of other correlated dietary components, such as fiber, which also...

Reaching beyond Researchers

Other resources explain particular age-related diseases. The Centers for Disease Control Cardiovascular Health site contains information for lay people such as fact sheets on topics that include cholesterol, heart attacks, and high blood pressure. Of particular interest to researchers are the interactive maps that supply heart attack and stroke mortality rates for the state, gender, and racial ethnic group of choice. The site includes a list of Morbidity and Mortality Reports that relate to cardiovascular disease as well as other statistical and public-health information. NIH's Osteoporosis and Related Bone Diseases-National Resource Center offers background articles on topics of interest to researchers as well as lay people these include ''Vitamin A and Bone Health,'' ''Phytoestrogens and Bone Health,'' and ''Bone Mass Measurement What the Numbers Mean.''

Klinefelter Syndrome Characteristics

Klinefelter syndrome Persons who have Klinefelter syndrome, which occurs with a frequency of about 1 in 1000 male births, have cells with one or more Y chromosomes and multiple X chromosomes. The cells of most males having this condition are XXY, but cells of a few Klinefelter males are XXXY, XXXXY, or XXYY. Persons with this condition, though male, frequently have small testes, some breast enlargement, and reduced facial and pubic hair ( FIGURE 4.11). They are often taller than normal and sterile most have normal intelligence.

Clinical manifestation

Gressive decline over the ensuing years (Mallon 2003). Peripheral fat loss can be accompanied by an accumulation of visceral fat which can cause mild gastrointestinal symptoms. Truncal fat increases initially after therapy and then remains stable (Mallon 2003). Visceral obesity, as a singular feature of abnormal fat redistribution, appears to occur in only a minority of patients. Fat accumulation may also be found as dorsocervical fat pads (buffalo hump) within the muscle and the liver. Female HIV patients sometimes complain about painful breast enlargement which has been attributed to the lipodystrophy syndrome. Whether gynecomastia in male patients is a component of the syndrome remains unclear. There is now accumulating evidence that the major clinical components - lipoatrophy, central adiposity and the combination of both - result from different pathogenetic developmental processes. The prevalence of lipodystrophy syndrome has been estimated to be between 30 and 50 based on...

Resection Specimens and the Types of Surgery

Tion margins increases local recurrence by a factor of 3.4. An extensive in-situ component increases local recurrence only when margins are involved. The presence of tumour lymphovascular invasion (LVI) doubles the local recurrence rate and Grade I tumours are less likely to recur locally compared to higher grade tumours. Some of these factors are only fully appreciated after detailed histological assessment and subsequent conversion to mastectomy is not an infrequent occurrence. Clinically, breast cancers which are suitable for treatment by breast conservation include a single clinical and mammographic lesion, tumour < 3 cm in diameter or > 3 cm in large breasts and with no sign of local advancement (i.e., no extensive nodal involvement or metastases).

Xenobiotic Pesticides

Both natural and synthetic environmental estrogens mimic the estrogenic activity of steroid hormones. Dietary phytoestrogens, as discussed above, have been proposed to act as estrogen antagonists in breast cells, potentially protecting these tissues from cancer formation. In contrast, xenobiotics, environmental estrogens such as pesticides, have been proposed to act as estrogen agonists, possibly increasing the risk of cancer formation. Although we would expect that a weak estrogen, whether from natural or synthetic sources, would act in a similar manner when applied to the same system, the effect is difficult to predict due to variable characteristics of the estrogen-like substances and the test systems. If the mechanism of action of environmental estrogens on cancer risk is strictly hormonal, it is unlikely that estrogenic pesticides and dietary phytoestrogens would exclusively produce opposite effects. Evaluation of in vitro and in vivo work for both phytoestrogens and xenobiotics...

Final Thoughts and Future Directions

As both fields continue to evolve both outside and within the law, we can expect jurors to confront new hybrids of science and technology in court. For example, in 1996 a chief judge in the US District Court of Northern Alabama appointed a neutral panel of four scientists (later referred to as the National Science Panel) to evaluate the scientific merit of claims that silicon breast implants cause chronic disease (Judson, 1999). The National Science Panel reported its findings in 1998 and submitted lengthy videotape depositions for use in subsequent breast implant litigation in federal courts (Breyer, 2000).

Anatomical And Morphological Relationships

In the first trimester of pregnancy, initial rapid growth and branching from the terminal portions of the gland are followed by the appearance of true glandular acini. The hollow alveolus becomes lined with a single layer of myoepithelial cells, ultimately enclosing the glandular alveolus in a loose network surrounded by capillaries so that the lumen of the alveolus is connected to an interlobular duct (see Figure 14-6). In the second trimester of pregnancy, the alveolar secretion begins. In the third trimester, particularly in the ninth month, breast enlargement occurs as a consequence of the hypertrophy of parenchymal cells and distention of the alveoli with colostrum.

Materials and Methods

Three-week old female ACI rats were obtained from Harlan Sprague-Dawley and immediately allowed free access to the AIN-76A diet (AIN) or AIN supplemented with 250 ppm GEN, their prepubertal diet. The AIN diet is a casein-based, open formula purified diet that contains nondetectable levels of phytoestrogens. At 9 weeks of age, they were subcutaneously implanted in the shoulder region with a 20 mg pellet containing either cholesterol or cholesterol as previously

Dietary Prostate Cancer Risk Factors

Phytoestrogens Phytoestrogens, i.e., plant-derived estrogens such as from soy products, have been hypothesized to protect against prostate cancer, possibly by antagonist actions of estrogens on androgen-mediated growth.19 There are some experimental animal data supporting an inhibitory effect of phytoestrogen intake on prostate tumor growth.19,20 Phytoestrogen intake has been hypothesized to play a role in the reduced prostate cancer rates observed in Asian populations. Cross-sectional population studies have shown urinary phytoestrogen biomarkers to be substantially higher in native Japanese than other racial ethnic groups, but there is little direct evidence linking high phytoestrogen consumption to low prostate cancer risk.21 One study found tofu consumption, a soy source of phytoestro-gens, to be associated with reduced risk but there is great need for further epidemiological investigations.22

What is Your Diagnosis

Breast implant Certain types of breast implants consist of a silicone rubber envelope filled with fluid. Until a few years ago, silicone gel was used exclusively. Now combinations of silicone gel and saline are used (saline in an outer lumen, silicone in the inner lumen of a double-lumen implant). After the implantation, a fibrotic capsule forms around the rubber envelope of the implant (Fig. 12.24a, b). The capsule often retains the fluid even after the rubber envelope has ruptured and collapsed (Fig. 12.24c).

Causes and symptoms

Delayed sexual maturation is a potential cause of HSDD. It is present in boys if there is no testicular enlargement by age 13-and-a-half or if there are more than five years between the initial and complete growth of the genitalia. In girls, delayed sexual maturation is characterized by a lack of breast enlargement by age 13, or by a period greater than five years between the beginning of breast growth and the onset of menstruation. Delayed puberty may be the result of familial constitutional disorders, genetic defects such as Turner's syndrome in females and Klinefelter's syndrome in males, central nervous system disorders such as pituitary conditions that interfere with the secretion of gonadotropic hormones, and chronic illnesses such as diabetes mellitus, chronic renal failure, and cystic fibrosis.

Chemical Components

Soy is a major food source of phytoestrogens (isoflavones and lignans), with each gram of soy protein containing approximately 1-3 mg of isoflavones, including glycosides of genistein, daidzein and glycitein (Erdman 2000). Soy also contains the lignans secoisolariciresinol, matairesinol, syringaresinol, lariciresinol, isolariciresinol, and pinoresinol (Penalvo et al 2004), as well as soy lecithin (a phospholipid containing 2007 Elsevier Australia

Oral contraceptives

Estrogens can cause nausea, breast tenderness and breast enlargement. Progestins can cause unfavorable changes in LDL and HDL cholesterol. Other adverse effects associated with oral contraceptives, such as weight gain or depression, are more difficult to attribute to one component or the other. Women smokers more than 35 years old who use combination oral contraceptives have an increased risk of cardiovascular disease.
